PHILADELPHIA, July 30, 2012 -- The American College of Physicians (ACP) and the Society of General Internal Medicine (SGIM) explore the ethical dimensions of the patient-centered medical home (PCMH) in a new position paper published by the Journal of General Internal Medicine: "The Patient-Centered Medical Home: An Ethical Analysis of Principles and Practice." The text is also available on ACP's website (http://www.acponline.org/running_practice/ethics/issues/policy/).

"The PCMH model of care aligns well with the traditional principles of medical ethics and professionalism, including the duty to promote the good and act in the best interest of the patient, the duty to do no harm to the patient, and respect for patient autonomy," said David L. Bronson, MD, FACP, president, ACP. "This position paper highlights some of the practical choices and implications of PCMH design and implementation that should be considered to ensure that this model of care becomes a key ingredient in better health care for patients."

In the paper, ACP and SGIM examine how the PCMH meets four fundamental ethical principles by facilitating:

  • a patient-centered approach to care, which reaffirms the core principles of medical ethics and professionalism
  • access to a personal physician who provides coordinated comprehensive care through an integrated team
  • involvement of patients, families, and caregivers in care, thereby supporting respect for patient wishes and autonomy
  • practice-based system improvement and explicit attention to quality

"The extent to which the practical implementation of the PCMH achieves ethical goals is likely to depend significantly on design features, such as the structure of physician payment and the measurement of patient satisfaction and experience, quality of care and patient outcomes, cost of care, and further health care reform," said Ann B. Nattinger, MD, MPH, president, SGIM.

Noting that many Americans are currently "medically homeless" and patients without access to care face a "perilous journey" as they try to navigate the health care system, the PCMH holds promise to substantively rectify this situation, bringing the health care system closer to the ideals of medical ethics and professionalism, ACP and SGIM say in the paper.

The organizations also note practical barriers to meeting some goals. For example, access to a personal physician responsible for coordination of care presents a challenge because of the shortage of primary care physicians.

ACP and SGIM say that the PCMH strongly supports the "bedrock principle" of patient autonomy because the concept of patient-centeredness that forms its foundation emphasizes patient engagement, provision of health information to patients, and involvement of patients in shared decision making.

By integrating system improvements into the practice environment, the PCMH could help physicians meet the ethical obligations for quality improvement and patient safety, ACP and SGIM say.

Man arrested after trying to hide gun in planter

(AP) ? An Oregon man has been arrested after airport authorities saw him trying to hide his gun in a planter box so he could retrieve it upon his return.

The Oregonian reports that 69-year-old Soren Muir Johnson of Eagle Point was arrested for investigation of reckless endangering.

Transportation Security Administration employees say he was told he couldn't carry his loaded weapon through a security checkpoint and that he should put it in his luggage instead.

They say that they watched as he disregarded that advice and instead dropped the .22-caliber pistol in an airport planter box on June 24.

Port of Portland spokesman Steve Johnson says police recovered the gun. Police say the man told them he hid the gun so he could retrieve it when he returned from his trip.

Chinese firm to buy Hawker Beechcraft

WICHITA, Kan. (AP) ? Struggling aircraft maker Hawker Beechcraft announced Monday that it had reached a $1.79 billion "exclusivity agreement" with a Chinese aerospace manufacturer for the sale of its business jet and general aviation operations in a deal that will save thousands of jobs in Kansas and Arkansas.

Under the agreement Beijing-based aerospace manufacturer Superior Aviation Beijing Co., Ltd., will buy Hawker Beechcraft and make payments over the next six weeks to support ongoing operations until the deal is finalized.

The sale does not include Hawker Beechcraft Defense Co., which will remain a separate entity.

If the transaction is completed, Superior intends to maintain Hawker Beechcraft's existing operations while putting "substantial capital" into the company and its business and general aviation product line, saving thousands of American jobs, the company said.

Hawker Beechcraft filed for bankruptcy protection in May to expedite a prearranged restructuring of the company and some of its subsidiaries. Hawker Beechcraft Corp., which is owned by GS Capital Partners and Onex Partners, has struggled with tepid demand for its military planes and business jets.

The company employs about 7,400 people, with roughly 4,700 working at its Wichita, Kan., facility. It also has factories in Little Rock, Ark., the U.K. and Mexico, as well as more than 100 service centers worldwide.

Steve Rooney, president of the local for the machinists union, did not immediately return a call for comment.

Robert Miller, CEO of Hawker Beechcraft Inc., said in a news release that Superior first approached Hawker Beechcraft years ago regarding a potential partnership. He cited Superior's "demonstrated ability to quickly restore a business to profitability after emerging from Chapter 11" and said a transaction with Superior would maximize value for Hawker Beechcraft and its stakeholders.

"Importantly, this combination would give Hawker Beechcraft greater access to the Chinese business and general aviation marketplace, which is forecast to grow more than 10 percent a year for the next 10 to 15 years," Miller said.

Bill Boisture, chairman of Hawker Beechcraft Corp., said Superior's bid for the company was the most attractive received during Hawker's strategic review. Its plan also offered the most continuity for Hawker Beechcraft, allowing the company to preserve jobs and product lines and maintain commitments to customers.

Under the agreement, Superior intends to make Hawker Beechcraft its flagship investment, maintain its U.S. headquarters, management team and employees and continue product development throughout its lines. If the parties negotiate a definitive agreement during the 45-day exclusivity period, it would be subject to an auction, Hawker said.

While neither ownership nor control of Hawker Beechcraft Defense Co., will transfer to Superior, up to $400 million of the $1.79 billion purchase price will be refundable to Superior in the event HBDC is sold.

The deal is also subject to regulatory approvals from the Chinese and U.S. officials. Bankruptcy court approval is also required.

If the deal collapses, Hawker Beechcraft said it will proceed with its bankruptcy reorganization plan to emerge as a stand-alone company.

Superior Aviation is 60 percent owned by Beijing Superior Aviation Technology Corp., Ltd, a closely held private entity, and 40 percent by Beijing E-Town International Investment & Development Corp., Ltd, a company controlled by the Beijing municipal government.

Since its founding and highly leveraged 2007 purchase of Raytheon Aircraft, Hawker Beechcraft has carried a heavy debt burden, reporting total debt of $2.3 billion at the end of 2011, according to its annual statement to the Securities and Exchange Commission.

Hawker Beechcraft said that the proposed combination of Hawker Beechcraft and Superior "will not require a financing condition."

In December 2010, Hawker Beechcraft struck a deal with state and local officials to keep its aircraft operations and at least 4,000 jobs in Kansas until 2020 in exchange for $45 million in incentives.

On Monday, the only comment from the Kan. Gov. Sam Brownback's office was a one-line response sent in an email: "The state is following the situation closely."

Also monitoring the situation are city leaders in Wichita, who also issued a brief statement saying they would work diligently to protect the city's investment and aviation jobs.

"At this time, the City is working to gain a better understanding of how the proposed acquisition may impact our community," Mayor Carl Brewer said. "We understand the financial challenges Hawker has faced and are optimistic this acquisition could preserve jobs and help Hawker avoid difficult financial decisions."

Hawker Beechcraft traces its Kansas roots to Beech Aircraft Corp., a company founded by Walter and Olive Ann Beech that began making aircraft in the 1930s.

Florida Football 100 For 100, No. 87: Sharrif Floyd, Florida's Lone Awards Hope?

Last year, we tried 50 For 50, and got halfway done before things cropped up. This year, Alligator Army is coming back with 100 For 100, with 100 articles in the 100 days leading up to the first Florida Gators football game of the 2012 season, and will run down storylines, profile players, examine history, and make some predictions, possibly all haphazardly. The only thing we can promise is that each day's entry will bring us all one step closer to "Herrrrrrrrre ... come the Gators!"

And maybe that you'll get a couple of these a day for the next week or so.

Remember when Tim Tebow was all over every awards list, and seemingly spent most of the Decembers of his last three seasons in Gainesvile at awards banquets? That's not so much the case today.

The Maxwell and Bednarik Award watch lists are both out today, and the only Gators player on either list is Sharrif Floyd, who makes the Bednarik list. Floyd is arguably the most talented player on Florida's roster, quick and powerful up front ? I know FlaGators really likes his talents, but I think he's still impaneled, and thus will not be able to back this up. But the chances Floyd has the sort of season that wins him the Bednarik Award seem slim to me.

There's significant stupidity inherent in preseason watch lists: It would be silly to name just a few players, and it's sillier still to name 65 players as "watch list" representatives, because that results in guys like Curtis McNeal (1,005 rushing yards for USC last year!) and Johnathan Franklin (you could have given me his conference and five guesses and I wouldn't have known what team he played for) being put on par with Montee Ball, Landry Jones, Denard Robinson, and Tyler Wilson.

But a team only having a few players on those watch lists says that few think that team has elite talent. It'll be up to Floyd and the Gators to prove otherwise.
